Disability Services Eligibility & Requirements

Reasonable accommodations and/or academic adjustments are approved upon completion of the Student Disclosure Form and either:

  • Submitting complete written documentation of a disability which clearly states the diagnosis or disability
    • 504 plan or IEP from high school
    • Psychological evaluation
    • Documentation from another college/institution
    • Clinical letter from a licensed professional whom you are currently a patient of
    • Paperwork from ACCES–VR or ABVI–Commission for the Blind
  • Consultation with Disability Services coordinator about what you are experiencing and how it impacts your learning

Note: Previous provision of services or lack thereof does not guarantee or preclude services at the college level.

    Note: This list is not inclusive of every diagnosis or disability that may qualify for services.

Handling of Records and Confidentiality

Documentation records related to diagnoses and disabilities are kept on file within the Disability Services office. We have the responsibility to maintain confidentiality of these records and will not release any such records without your written consent.

Students are not required to reveal the nature of their disability to an advisor or instructor. Students must share their Accommodation Notification with instructors in order to begin utilizing the accommodations and adjustments that have been approved. The Accommodation Notification does not state personal information or reveal any details about a diagnosis or disability.
