Who Should Use this Form?

Former degree-seeking students planning to re-enter a degree program after being away from Finger Lakes for six years or less should complete our re-matriculation application form.

You will receive a notification by email regarding the status of this application. If you are applying to the nursing program, you will hear about this application from the Admissions Office. Completion of this form does not constitute formal acceptance into the program you indicate below.
